When you come in for your initial visit, please plan to arrive 10-15 minutes early to complete your new patient registration forms, and be sure to bring your dental insurance card. Please bring a list of any medications you are taking. If you need antibiotics before dental care, please take them as prescribed. If you are not sure if you need antibiotics, please get in touch with your doctor prior to your visit.
